Home Health Aide

Home Health Aides and CNAs Provide personal care services obtained in a written assignment by and under the supervision of a Registered Nurse or Therapist. Personal care services may include assisting patients with personal hygiene, ambulation, eating, dressing, grooming, shaving, and personal transfer.

Registered Nurse

Nursing Services are provided by Registered Nurses (RNs) and Licensed Practical Nurses (LPNs). An RN is the case manager in all cases involving nurses or both nursing and and therapy care. The RN furnishes services requiring substantial and specialized nursing care such as Intravenous Therapy, Wound Care Management, Disease Management on chronic diseases such as Congestive Heart Failure, Diabetes, Hypertension, and Chronic Obstructive Pulmonary Disease. These services help in reducing emergency care, and prevent frequent re hospitalization.

Medical Social Worker

Assist patients and caregivers with personal and environmental difficulties, which predispose toward illness or interfere with obtaining maximum benefits from medical care. Medical social workers identify resources such as caregivers such as community agencies to assist patients in resuming life in the  community or learn to live with the disability.

Physical Therapist

Provide rehabilitative services such as strengthening, transferring and gait training. Occupational Therapist

Guide Patients in the use of therapeutic, creative, and selfcare activities in improving their independence in activities of daily living. Occupational Therapists also provide education and guidance to the patient’s caregivers and other team members.
